Becoming appointed with Priority Health

Thank you for your interest in Priority Health. To become appointed with us, you'll need to fill out an agent agreement. Choose 1 based on the Priority Health products you'd like to sell:

• Group (commercial) health plans agent agreement
• Individual health plans (MAPD, Medigap and MyPriority) agent agreement 

Note: If you'd like to sell both group and individual plans, fill out both agent agreements.

Group (commercial) health plans agent agreement

1. Download the group health plans agent agreement (614KB PDF).
2. Fill out the agreement. Use these pointers:
   • Page 1 - Fill in your (or the agent's) name and primary place of business. Priority Health contracts with individual agents, not agencies as a whole.
   • Page 3 - Complete the "Commission checks payable to" section. We can pay commissions to you or your agency.
   • Page 7 - Make sure you fill in all of your information and sign this page
   • W9 - Make sure the W9 matches page 3. Enter agency information if commission payments go to agency. Enter your information if commission payments go to you.
3. Return the completed agreement to the agent services coordinator at:

Once we receive your agreement

We'll send you a confirmation email with your broker number and a username so you can access your online Agent Center.

Individual health plans (MAPD, Medigap, MyPriority) agent agreement

You need to complete a Priority Health individual plans agent agreement in order to sell Priority Health Medicare Advantage*, Medigap and/or MyPriority plans.

*If you want to sell Medicare Advantage (MAPD) plans, you'll also need to complete certification training. Get more information below.

See instructions:

1. Download the individual plans agent agreement (885KB PDF).
2. Fill out the agreement. Use these tips:
   • Page 1 - Fill in the agent's name and primary place of business. Priority Health contracts with individual agents, not agencies as a whole. 
   • Page 9 - Fill in agent information.
   • Attachment B - Complete the Assignment of Commissions. We can pay commissions to the individual agent, general agent, field marketing organization or agency. 
   • W9 - Make sure the W9 matches the Assignment of Commission. If commission is paid to the agency or general agent, fill in agency or general agent information. If commission is paid to the individual agent, fill in the agent's information. 
3. Return the completed agreement to the agent services coordinator at:

Getting approved

Medigap and MyPriority

We'll send you a confirmation email with your broker number and a username so you can access your online Agent Center.

Priority Health Medicare Advantage

You must complete certification training before you'll be approved to sell Priority Health Medicare Advantage plans. Training includes 2 steps: online training and a classroom session.
