One source for your Medicare. Priority Health provides your Medicare Parts A (hospitalization), B (doctor visits) and D (prescription drug coverage) in a single plan for a single premium. Note: You still have to pay your Medicare Parts A and B premiums, if they apply to you.

Your Priority Health group Medicare plan documents
- Your plan year may not run from January 1 to December 31.
- Your employer customizes your plan and gives you a Summary of Benefits brochure and your Evidence of Coverage (the booklet that contains everything about how the plan works).
- If you need replacements for your plan documents or have questions about your coverage, call our Medicare plan customer service representatives.
Finding doctors, hospitals, pharmacies, and other health care providers
Use the Priority Health Medicare plan provider network. Group plan members use the same network of health care providers as people who buy our individual Priority Health Medicare plans.
