HealthbyChoice ProgressionsSM combines your medical plan (PriorityHMOSM, PriorityPOSSM or PriorityPPOSM) with the opportunity to pay less when you stay healthy or when you make measurable improvements to your health.
HealthbyChoice Progressions plan features
- Two cost levels: Standard and Choice.
- Everyone starts at the Standard level.
- You'll pay a lower deductible, lower copays and lower coinsurance if you earn the Choice level.
- The same medical services are covered no matter which cost level you have.
- If your spouse or domestic partner is covered by your plan, you both must meet the requirements to earn the Choice level.
- Once you earn the Choice level, you must re-qualify for it every 12 months.
